TEG应用于急性脑梗死患者凝血功能动态监测价值分析

中文摘要: 目的 探讨血栓弹力图(TEG)应用于急性脑梗死患者凝血功能动态监测的意义。方法 选取我院2016年4月—2016年12月800例急性脑梗死患者作为此次研究对象,根据患者神经功能缺损评分情况分组为甲组(轻度)、乙组(中度)和丙组(重度),分别为322例、278例和200例。同时选取同期入院健康体检的200例健康体检者作为此次研究对照组。 检测各组TEG参数和血浆凝血酶原时间(PT)、D-二聚体(DD)、部分凝血活酶时间(APTT)和血浆纤维蛋白原(FIB),并进行分析。结果 急性脑梗死患者R、K分别为(5.98±1.67) min、(1.08±0.31) min明显低于对照组(6.82±2.19) min、(2.25±0.61) min,P <0.05;急性脑梗死患者α角、MA分别为(69.47±6.72) °、(69.57±5.14) mm,明显高于对照组(59.81±5.87) °、(59.88±4.58) mm,P<0.05;然两组CI相比较,P >0.05。急性脑梗死患者DD、FIB水平分别为(0.42±0.17) mg/L、(4.57±0.69) g/L,明显高于对照组(0.23±0.07) mg/L、(2.58±0.79) g/L,P <0.05;然两组PT、APTT相比较,P >0.05。甲乙丙三组患者间R、K、α角、MA、DD和FIB相比较,P <0.05。R值、K值与DD、FIB呈负相关性,与PT、APTT呈正相关性,P <0.05;α角、MA及CI则与DD、FIB呈正相关性,与PT、APTT呈负相关性,P<0.05。结论 急性脑梗死患者存在凝血功能障碍,TEG与患者凝血功能存在一定相关性,因此通过检测TEG和凝血功能可有助于临床医师评估患者病情及预后。
Abstract:OBJECTIVE To explore the significance of thrombus elastography (TEG) in the monitoring of coagulation function in patients with acute cerebral infarction.METHODS In our hospital from April 2016 to December 2016, 800 patients with acute cerebral infarction were selected as the subjects. According to the score of neurological deficit, the patients were divided into group A (mild), group B (moderate) and group C (severe), respectively, 322 cases, 278 cases, 200 cases. At the same time, 200 healthy subjects were selected as the study control group. The levels of TEG and plasma prothrombin time (PT), D-dimer (DD), partial thromboplastin time (APTT) and plasma fibrinogen (FIB) were measured and analyzed.RESULTS (5.98±1.67) min, (1.08±0.31) min, respectively, which were significantly lower than those in the control group (6.82±2.19) min, (2.25±0.61) min, P<0.05; acute cerebral infarction patients (69.87±6.72) mm, (69.57±5.14) mm, which was significantly higher than that of the control group (59.81±5.87) °, (59.88±4.58) mm, P<0.05. The levels of DD and FIB in patients with acute cerebral infarction were (0.42±0.17) mg/L and (4.57±0.69) g/L, respectively, which were significantly higher than those in the control group (0.23±0.07) mg/L, (2.58±0.79) g/L , P<0.05; then the two groups PT, APTT compared, P>0.05. The levels of R, K, α, MA, DD and FIB were significantly higher in group A and C (P<0.05).. A, MA and CI were negatively correlated with DD and FIB, and negatively correlated with PT and APTT, and there was a positive correlation between PT and FIB. P<0.05.CONCLUSION TEG and coagulation function can help clinicians to assess the condition and prognosis of patients with acute cerebral infarction.
