中文摘要: 目前，社区慢性病管理工作存在效率不高、效果欠佳，对疾病危险因素的综合性干预不足的问题。针对上述情况，有必要结合当前推行的家庭医生制对当前社区慢性病管理模式进行优化和创新。因此，本文提出应充分发挥家庭医生优势，促进社区卫生服务同质化发展。并且建议建立以家庭医生为主导的社区慢性病综合管理新模式，按照疾病风险等级分层开展疾病防治工作，建立慢性病管理绩效考核新机制，着力提升居民知、信、行水平，加强信息化智能辅助功能建设。
Abstract:The community management of chronic diseases is deficient effect，and insufficient comprehensive intervention on the risk factors of disease. In view of the above situation, it is necessary to optimize and innovate the community management model of chronic diseases under the family doctor system. Therefore, this paper proposes that we should give full play to the advantage of family doctors，and promote the homogenization of community health services. It is recommended to establish a new integrated management model of chronic diseases dominated by family doctors，carry out disease prevention and control according to the risk level of disease，establish new mechanism for managing performance of chronic diseases，improve the level of residents' cognition，strengthen the construction of informatization intelligent auxiliary functions.
