依帕司他在早期糖尿病肾病患者辅助治疗中的作用

中文摘要: 目的 分析依帕司他在早期糖尿病肾病(DN)患者辅助治疗中的作用。方法 选取早期DN患者94例,随机分为治疗组和对照组,每组各47例。在常规治疗基础上,对照组予以前列地尔,治疗组予以前列地尔联合依帕司他治疗,疗程均为3个月。观察治疗前后两组肾功能、过氧化损伤及炎症损伤相关指标的变化。结果 治疗后,两组血清肌酐(Scr)、胱抑素C(CysC)、血尿素氮(BUN)及β2微球蛋白(β2-MG)水平较治疗前明显降低(P<0.01),但治疗组降低幅度更显著(P<0.05);两组活性氧(ROS)、晚期蛋白氧化产物(AOPPs)及降钙素原(PCT)、白细胞介素-8(IL-8)、IL-27水平较治疗前明显降低,过氧化氢酶(CAT)、总超氧化物歧化酶(TSOD)及IL-4、IL-10和IL-13水平明显升高(P<0.01),但治疗组降低、升高幅度更显著(P<0.05)。结论 采用依帕司他辅助治疗早期DN可有效保护患者肾功能,缓解全身氧化应激程度及炎症损伤。
Abstract:OBJECTIVE To analyze the effects of Epalrestat in adjuvant treatment of patients with early diabetic nephropathy (DN). METHODS A total of 94 patients with early DN were selected and randomly divided into treatment group and control group, 47 cases in each group. On the basis of routine treatment, control group was added with Alprostadil while treatment group with Alprostadil concomitant with Epalrestat, for 3 months in total. The changes of renal function, peroxidation damage and inflammatory damage related indexes before and after treatment were observed in both groups. RESULTS After treatment, serum creatinine(Scr), cystatin C(CysC), blood urine nitrogen (BUN) and β2 microglobulin (β2-MG)levels were decreased notably in both groups than treatment before(P<0.01), with decrease amplitude being more significant in treatment group(P<0.05). Reactive oxygen species (ROS), advanced oxidation protein products(AOPPs), procalcitonin(PCT), interleukin(IL)-8 and IL-27 levels were decreased prominently while catalase(CAT), total superoxide dismutase (TSOD), IL-4, IL-10 and IL-13 levels were increased apparently in both groups than treatment before(P<0.01), with decrease and increase amplitudes being more significant in treatment group(P<0.05).CONCLUSION Adjuvant treatment with Epalrestat can effectively protect the renal function and relieve the systematic oxidative stress severity and inflammatory damage in patients with early DN.
