中文摘要: 目的 了解浦东新区社区卫生服务中心内员工的整体心理状况,探索缓解医务人员心理压力、提升心理健康水平、掌握自我情绪管理能力的效果。方法 选取上海市浦东新区3家社区卫生服务中心,采用GHQ-12、SAS、SDS和MBI-GS等量表评估中心内员工的心理状况,对存在焦虑、抑郁或职业倦怠者开展团体心理治疗,以与干预人群基本情况相匹配且无心理问题的医务人员作为对照组,评价干预效果。 结果 机构员工12项一般健康问卷总分为(22.17±5.24)分,焦虑自评量表标准分为(40.67±10.82)分,抑郁自评量表标准分为(41.14±11.95)分,Maslach职业倦怠调查普适量表总分为(19.79±15.77)分。团体心理治疗干预前两组人群在SAS、SDS、GHQ-12以及MBI-GS量表各因子中均具有显著差异(P﹤0.01)。干预后两组人群在SDS、MBI-GS的情绪衰竭、工作态度、总均分/总分等因子中仍存在显著差异(P﹤0.01),其余量表与因子则无差异。结论 基层社区卫生服务中心员工的心理状况与普通人群接近,医疗业务、预防保健、行政等科室以及工龄较高且职称较低的员工是心理弱势群体。团体心理治疗能够明显改善心理异常员工的焦虑并提升其成就感。
Abstract:OBJECTIVE To master the overall psychological status of the staff in Pudong New Area community health service center, and to explore the effect of relieving the psychological pressure of medical staff, improving the level of mental health, and mastering the ability of self emotion management. METHODS Select 3 community health service centers in Pudong New Area, Shanghai, GHQ-12, SAS, SDS and MBI-GS were used to evaluate the staff of Community Health Service Center, Group psychotherapy for those with anxiety, depression or job burnout. The medical staff who matched the basic situation of the Population and had no Psychological Problems were used as the control group. The effect of the intervention method was evaluated. RESULTS The total GHQ-12 score of the staff was 22.17±5.24, The standard of SAS was (40.67±10.82) scores .The standard of SDS was (41.14±11.95) scores. The total score of MBI-GS was (19.79±15.77) scores. The scores of SAS, SDS, GHQ-12 and MBI-GS in different department employees were obviously different (P<0.01). There was a Positive correlation between the working years and the score of SAS, SDS, GHQ-12, emotional exhaustion, working attitude in MBI-GS. There was a negative correlation between Professional title and the score of GHQ-12, the sense of achievement in MBI-GS. The scores of SAS, SDS, GHQ-12, and MBI-GS scales were significantly different in the two groups before group Psychotherapy(P﹤0.01). After group Psychotherapy, the scores of emotional exhaustion, work attitude, total and total score in SDS and MBI-GS of the two groups were still different. (P﹤0.01), There was no difference between the rest of the scale and the factor obtained by the factor. CONCLUSION The Psychological status of the staff in the community health service center is similar to that of the ordinary People. The disadvantaged People include the Department of clinical medicine, the Public health department, the staff in the administrative department and an employee with high working age and low title. Group psychotherapy can significantly improve the anxiety of abnormal mental workers and enhance their sense of achievement.
