Abstract:Objection In order to make action by the Long-term plan for chronic diseasepreven- tion and treatment, we investigate the resource and works of the chronic disease management in primary care institutions in Haikou and analysis the bottleneck in doing this works. Methods we use the table designed by own to survey the 130 primary care institutions in haikou, the survey content including the human resource, management in chronic disease and support environment. Results The management staff is few in primary care institutions which major mainly are clinical and nursing, pre- staff serve 293 patients with chronic diseases, and the management activities are not standardized. The frequency of information updating of chronic patients was low. There were 61 primary care institutions without no pay for performance . which most of the them are owner of township government or community health service stations . There is a large varied funding in chronic disease management between the primary care institutions in Haikou , most of which is about 120,000 yuan, compared with the management of patients with chronic diseases, the funds are serious insufficient. The follow-up the chronic patients compliment was low. Conclusion The less of funds for chronic disease management in primary care institutions in Haikou has threated the activity sustain, and the poor pay for performance management system restricts the development of chronic disease management.
