中文摘要: 目的 对脓毒症致急性肾损伤(AKI)患者采用连续性肾脏替代疗法(CRRT)治疗的临床效果进行探析。方法 选取2016年1月—2018年1月周口市中心医院收治的102例脓毒症致AKI患者作为观察对象,通过数字表法随机分成观察组和对照组各51例;两组均给予对症处理,对照组采取常规间歇性血透(IHD)治疗,观察组采取CRRT治疗,测定两组治疗前后的C-反应蛋白(CRP)、血肌酐(SCr)、血尿素氮(BUN)及尿量改善情况,并掌握尿量恢复时间、ICU治疗时间及心血管事件发生率。结果 治疗后,两组患者的CRP、SCr、BUN及尿量均有改善,但观察组效果更优于对照组(P<0.05);观察组尿量恢复时间、ICU住院时间短于对照组,差异有统计意义(P<0.05);观察组心血管事件发生率为3.92%,显著低于对照组的15.69%(P<0.05)。结论 采用CRRT治疗脓毒症致AKI效果确切,能有效清除炎性介质,改善患者肾功能,减少心血管事件发生,临床意义重大。
Abstract:Objective To explore the clinical effect of continuous renal replacement therapy (CRRT) in patients with acute kidney injury (AKI) caused by sepsis. Methods 102 patients with AKI caused by sepsis treated in our hospital from January 2016 to January 2018 were selected as observation objects. They were randomly divided into observation group and control group with 51 cases in each group by digital table method. Both groups were given symptomatic treatment. The control group was given routine intermittent hemodialysis (IHD) treatment. The observation group was given CRRT treatment. The C-reactive protein (CRP), serum creatinine (SCr), blood urea nitrogen (BUN) and urine volume were measured before and after treatment. The recovery time of urine volume, the time of ICU treatment and the incidence of cardiovascular events were mastered. Rate of birth. Results After treatment, CRP, SCr, BUN and urine volume of the two groups were improved, but the effect of the observation group was better than that of the control group (P<0.05). The recovery time of urine volume and hospitalization time of ICU in observation group were shorter than those in control group (P<0.05). The incidence of cardiovascular events in the observation group was 3.92%, which was significantly lower than 15.69% in the control group (P <0.05). Conclusion CRRT is effective in treating AKI caused by sepsis. It can effectively remove inflammatory mediators, improve renal function and reduce cardiovascular events. It is of great clinical significance.
