Abstract:Abstract Internet plus was an effective means to enhance the chronic disease management ability of primary health care institutions. Project management had the advantage of concentrating on major events. As the main force of chronic disease treatment and management, the family doctor team broke through the conventional diabetes management framework, and carried out telemedicine management for diabetic patients using insulin in the form of project management. It constructed and applied the whole process of diabetes telemedicine management from the aspects of planning, task decomposition, team work division, technical specifications, informed consent, management process, quality control and result evaluation. After 12 months of telemedicine management, the HbA1c, blood glucose control rate and the satisfaction rate of contracted services of 100 diabetes patients treated with insulin in target community were higher than those before. The family doctor team provided diabetes telemedicine management, which had high promotion value on the basis of strengthening quality control and improving the legal system.
