Abstract:Objective To explore the control, leverage, problem, vulnerability of the practice of the “treatment-care and four linkage combination” integrated care model in Hefei city, and provide a reference for further optimizing the application effect of this model in the cooperation of medical association units. Methods The SWOT-CLPV model was used to analyze the implementation status and remaining problems of the “treatment-care and four linkage combination” integrated care model.Results Hefei City has taken the lead in adopting the “treatment-care and four linkage combination”integrated care model. Through the close connection between hospitals, medical institutions, communities, and families, it has formed a multi-directional flow of medical resources and promoted communication and collaboration between medical federation units. And the practice of the model has provided more convenient channels for the elderly to seek medical care.Conclusion In the practice of the “treatment-care and four linkage combination”integrated care model,Hefei City has improved the cooperation mechanism between medical consortia units, and the mobility of medical care and elderly care services is better. Medical and health units at all levels need to enrich the implementation of the quaternary linkage model,grasp the current situation of the old-age care policy, meet the elderly-healing needs of the elderly in the region of Hefei and promote the efficient operation of the “treatment-care and four linkage combination”integrated care model.
