以标化工作量为基础的基层医疗机构绩效管理*

中文摘要: 近期,天津市全面推进基层医疗卫生机构标准化全科门诊建设, 采取科室整合、服务流程再造等措施,推广预约就诊、定向分诊、诊前健康管理服务、诊间履约服务和诊后复诊预约的标准化服务流程。流程改造带来了家庭医生团队成员之间的服务协作和医疗、预防及康复服务之间的融合,概括来说是“先服务,后诊疗”的服务模式。这种服务模式对“以经济收入”为主要参考的绩效管理方式提出了挑战。因此,将标化工作量为基础考核“服务”岗位与“诊疗”岗位绩效,对调动了广大医务人员工作的积极性具有重要意义。本研究将基层医疗机构工作岗位(除部分行政岗位)分为一般诊疗类项目、健康管理类项目和医技检验类项目,根据项目的不同特色进行标化,为服务模式转型后的基层医疗机构绩效管理提供借鉴。
Abstract:Abstract Recently, the Standardized General Outpatient Service Process has been promoted in grassroots health service institutions in TianJin. It adopts the measures of clinical department integration and service process reengineering to make the service process according to the order of Appointment -- Triage -- Pre-consultation health management service -- Inter-consultation performance service -- Post-consultation follow-up appointment. The process transformation has brought about service collaboration among family physician team members and integration of medical, preventive and rehabilitation services. In a word, it is the service mode of "service first, diagnosis and treatment later". This kind of service mode challenges the performance management mode with economic income as the main reference.Therefore, taking standardized workload as the basis to evaluate the performance of "service" posts and "diagnosis and treatment" posts is of great significance to mobilize the enthusiasm of the majority of medical personnel. In this study, the posts of primary medical institutions (except some administrative posts) were divided into general diagnosis and treatment projects, health management projects and medical technology inspection projects, and standardized according to different characteristics of the projects, providing reference for the performance management of primary medical institutions after the transformation of service mode.
