Abstract:OBJECTIVE To explore the management mode of community peritoneal dialysis,improve the management efficiency and improve the treatment effect. METHODS 52 patients with peritoneal dialysis in Anting community of Jiading District were selected as intervention group,74 patients from other communities served as control group.Patients in the control group were followed up routinely, while those in the intervention group were given community peritoneal dialysis management mode with family doctor as the core, including operation guidance and health education. After 2 years of intervention, the effect of the management was evaluated. RESULTS The self-management behavior, quality of life and blood biochemical indexes of intervention group were significantly improved compared with those before intervention(P<0.01), and the indexes of the intervention group were significantly improved compared with those of the control group after the intervention (P<0.01).The incidence of peritonitis, tunnel mouth infection and catheter displacement in the intervention group was significantly lower than that in the control group(P<0.01).The dialysis adequacy rate of intervention group was significantly higher than that of control group (P<0.01), and The mortality rate and the rate of entering hemodialysis were significantly lower than those of the control group (P<0.01).CONCLUSION Community peritoneal dialysis management centered on family doctors can effectively delay the progress of kidney disease, improve prognosis and improve the quality of life of patients,which is of great significance for promotion.
