中文摘要: 目的 探索和实践以信息化为核心的糖尿病眼病综合防治管理模式的效果评估。方法选取上海市闵行区糖尿病眼病电子建档和管理人群作为研究对象，分年段比较糖尿病眼病建档率、远程诊断率、复诊率、规范管理率、糖尿病视网膜病变(DR)筛查发现率和DR患者治疗率。结果 2017—2019年糖尿病眼病建档率、远程诊断率、复诊率和规范管理率逐年提高，至2019年底，累计建档29 683份，建档率52.63%；远程诊断15 988例，远程诊断率93.10%；复诊146例，复诊率6.40%；一般管理对象和重点管理对象规范管理率分别为34.60%和37.61%，DR筛查发现率48.46%，DR患者治疗率0.57%。结论闵行区已构建成以信息化为支撑、远程筛查技术为手段和形成“筛查—发现—转诊—随访—健康管理”医防紧密融合的糖尿病眼病综合防治管理模式。
Abstract:OBJECTIVE To explore and practice the evaluation effects of diabetic eye disease management model with informationization as the core. METHODS The patients with electronic establishment and management of diabetic eye diseases in Minhang District, Shanghai were selected as the research object. The electronic health records rate, remote diagnosis rate, rate of review, standardized management rate of diabetic eye diseases, diabeticretinopathy(DR) screening discovery rate and DR patient treatment rate were compared. RESULTS From 2017 to 2019, the electronic health records rate, remote diagnosis rate, follow-up rate, and standardized management rate of diabetic eye diseases increased year by year. By the end of 2019, 29 683 files were established in Minhang District, with a file rate of 52.63%; 15 988 cases were remotely diagnosed, with a remote diagnosis rate of 93.10%; 146 cases were re-diagnosed, with a follow-up rate of 6.40%; the standard management rate of general management objects and key management objects were 34.60% and 37.61% respectively; the discovery rate of DR screening was 48.46%, the treatment rate of DR patients was 0.57%. CONCLUSION A comprehensive medical & preventive management model of diabetic eye disease including the whole process of “screening-discovery-referral-follow up-health management”, with the support of information technology and remote screening technology has been established in Minhang District.
