Abstract:According to the key elements of ICCC framework, qualitative interview data and literature data was collected to analyze the practice of hypertension health management in National Medical Alliance for Hypertension, summarize the characteristics and shortcomings. The study found the hypertensive health management in National Medical Alliance for Hypertension ran well on the whole, with all the elements at the macro level, while some elements at the meso and micro levels were still lacked. Based on the ICCC framework, it is suggested that the role of communities should be strengthened. A more cooperative system between healthcare institutions and CDCs should be established. The incentive mechanism for healthcare personnel should be improved and needs of patients at different levels should be met to reach a better healthcare quality. Through taking above measures, the purpose is to form a well-managed and efficient hypertension health management model, and achieve the best outcome of chronic disease prevention and control.
