中文摘要: 摘 要 目的:基于北京市平谷区社区卫生服务机构常规监测数据,分析 2015—2019年平谷区家庭医生签约服务政 策及服务提供等情况,分析签约服务工作概况和存在的问题,并提出政策建议。方法:数据来源于 2015—2019年北京市平 谷区 18家社区卫生服务中心和 127个社区卫生服务站的常规监测数据;采用描述性统计方法进行分析。结果:2015—2019 年,平谷区常住人口以及60岁以上老年人、0~6岁儿童和孕产妇等重点人群数均逐年增加。在签约服务人才队伍方面,全科 医生、社区护士、防保人员及其他人员年均增长率分别为1.2%、0.9%、3.9%和13.6%,但是,签约服务人员增长速度低于重 点人群增长速度,全科医生、社区护士、防保人员在签约服务团队中占比逐年下降。平谷区全人群签约率从 26.5%上升至 34.8%;重点人群签约数从71942人增长至108891人,年均增长率6.1%,重点人群签约率从83.3%上升至93.0%。平均每名 全科医生签约人数从424人增长至580人,签约重点人群数从273人增长至393人,全科医生人均负荷逐年增加,效率呈上升 趋势。签约总人数年均增长率为9.4%,续约率从40.7%增长至94.8%,新签约人数占比从40%降至6.2%,表明签约服务人群 不断稳定。结论:北京市平谷区家庭医生签约服务数量与质量均呈提升态势,居民认可度逐年提升;医生签约服务人均负荷 逐年增加,效率逐年提升,但人才队伍建设相对滞后;下一步需继续提升家庭医生签约服务能力并完善激励机制。

Abstract:OBJECTIVE To evaluate the progress of family doctor contract services of a rural area in Beijing to propose policy recommendations for local government in China based on the regular monitoring data of primary care facilities of Beijing. METHODS The data was collected from the regular monitoring data of 18 community health service centres and 127 community health service stations in Pinggu District of Beijing from 2015 to 2019. RESULTS From 2015 to 2019,the resident population of Pinggu District,the number of people over 60 years old,children 0~6 years old and pregnant and lying-in women increased year by year. The health staff for contract service delivery were increasing,with annual growth rates of general practitioners (GPs),nurses,public health staff and other staff as 1.2%,0.9%,3.9% and 13.6%. However,the growth rates of health staffs were lower than that of the population. In addition,the share of health technical personnel kept decreasing during 2015 to 2019,which increased the burden of family doctors. The signing rate of the whole population in Pinggu district increased from 26.5% to 34.8% and the number of key
groups increased from 71 942 to 108 891 with an average annual growth rate of 6.1%. The signing rate of key groups increased from 83.3% to 93%. The average number of people signed increased from 424 to 580 and the number of key groups increased from 273 to 393. The per capital load of general practitioners and the efficiency were increasing year by year. The average annual growth rate of the total number of signed contracts was 9.4%,the contract extension rate increased from 40.7% to 94.8% and the proportion of new signings decreased from 40% to 6.2%. It indicated the continuous stability of the contracted service population. CONCLUSION The quantity and quality of family contract service were increasing from 2015 to 2019 and the family doctor contract service got into a stable stage. The efficiency of service delivery kept improving while the health staff became relatively insufficient. The major task for the next step was to improve the capacity and quantity of health staff of primary care system and further stimulate the health staff.
