基质金属蛋白酶9及其组织抑制因子1在社区获得性 肺炎和肺结核鉴别诊断中的价值*

中文摘要: 摘 要:目的:探讨基质金属蛋白酶 9 (MMP-9) 及基质金属蛋白酶组织抑制因子 1 (TIMP-1) 在社区获得性肺炎 (CAP)和肺结核(PTB)鉴别诊断中的价值。方法:回顾性收集2018年6月—2021年3月河南省胸科医院收治的社区获得性 肺炎患者156例作为肺炎组,收治的肺结核患者98例作为肺结核组,比较分析两组患者血清MMP-9、TIMP-1水平及其比值 差异。采用受试者操作工作曲线 (ROC) 分析 MMP-9、TIMP-1 水平及其比值的鉴别诊断价值。结果:肺结核组 MMP-9、 TIMP-1水平及其比值均显著高于肺炎组,差异均有统计学意义 (t值分别为5.573、3.362、7.116,P<0.05)。进一步相关性 分 析 发 现 , MMP-9 水 平 与 MMP-9/TIMP-1 呈 正 相 关 (r=0.143, P=0.023), TIMP-1 水 平 与 MMP-9/TIMP-1 呈 负 相 关 (r=-0.129,P=0.040),且三者鉴别社区获得性肺炎和肺结核的ROC曲线下面积 (AUC) 分别为0.702、0.645、0.756。结论: MMP-9、TIMP-1水平及MMP-9/TIMP-1在社区获得性肺炎和肺结核患者中存在差异,MMP-9及MMP-9/TIMP-1对社区获得 性肺炎和肺结核具有一定鉴别诊断价值。
Abstract:Abstract: OBJECTIVE To investigate the value of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metallopoteinase 1(TIMP-1) in the differential diagnosis of pulmonary tuberculosis (PTB) and community-acquired pneumonia (CAP). METHODS 156 cases of CAP children (CAP group) and 98 cases of PTB children (PTB group) who admitted to the author’s hospital from June of 2018 to March of 2021 were retrospectively collected, and the differences in serum MMP-9 and TIMP-1 levels and their ratios be⁃ tween the two groups were compared and analyzed. Receiver operating characters curve (ROC) was used to analyze the differential di⁃ agnostic value of MMP-9 and TIMP-1 levels and their ratios. RESULTS Compared with CAP group, MMP-9 and TIMP-1 levels and the ratios were significantly increased in PTB group. The differences were all significant (t=5.573, 3.362, 7.116, P<0.05). Further correlation analysis found that MMP-9 levels were positively correlated with MMP-9/TIMP-1 (r=0.143, P=0.023), and TIMP-1 levels and MMP-9/TIMP-1 (r=-0.129, P=0.040), and the ROC area (AUC) was 0.702, 0.75, 0.645 for identifying community-ac⁃ quired pneumonia and tuberculosis, respectively. CONCLUSION The levels of MMP-9, TIMP-1 and MMP-9/TIMP-1 were different between CAP and PTB, and MMP-9 and MMP-9/TIMP-1 have certain diagnostic value between CAP and PTB.
