中文摘要: 目的 探讨听觉事件相关电位(AERPs)在轻度认知功能障碍(MCI)诊断中的价值。方法 选取艾尔兹海默病(AD)患者20例为AD组，轻度认知功能障碍(MCI)患者37例为MCI组，及健康体检者35例为对照组，行AERPs检测。结果 AD组计数准确率显著低于MCI组及对照组，差异有统计学意义(P＜0.05)；AD组反应时间显著长于MCI组及对照组，差异有统计学意义(P＜0.01)；MCI组与对照组计数准确率、反应时间均无显著差异(P＞0.05)。MCI组、AD组P300潜伏期较对照组相比，明显延长，波幅降低；MCI组与AD组比较，P300潜伏期缩短、波幅增高(P＜0.05，P＜0.01)；3组N200潜伏期、波幅无显著变化(P＞0.05)。结论 N200对反映MCI及AD的认知功能障碍方面不如P300敏感，P300能客观反映大脑认知和判断功能，其异常变化程度可为MCI诊断提供依据。
Abstract:OBJECTIVE To explore the value of auditory event-related potentials (AERPs) in the diagnosis of mild cognitive impairment (MCI). METHODS A total of 20 patients with Alzheimer's disease (AD) served as AD group while another 37 with MCI as MCI group. A total of 35 healthy people undergoing physical examinations were selected as normal control group. AERPs detection was conducted to all groups. RESULTS AD group was evidently lower than MCI group in the counting accuracy rate, and there was significant difference (P＜0.05). AD group was markedly longer in reactive time (RT) than MCI group and control group (P＜0.01). However, there was insignificant difference between MCI group and NC group in counting accuracy rate and RT (P＞0.05). Both MCI group and AD group was evidently longer in incubation period of P300 and lower in wave amplitude of P300 than control group, and MCI group was prominently shorter in incubation period and notably higher in wave amplitude of P300 than AD group (P＜0.05 or P＜0.01). However, there was insignificant difference in the changes of incubation period and wave amplitude of N200 in all groups (P＞0.05). CONCLUSION N200 is less sensitive than P300 in reflecting MCI and cognitive impairment in AD patients as P300 can objectively reflect the cerebral cognition and judge function, and its abnormal change degree can provide basis for the diagnosis of MCI.
