经颅多普勒超声联合颈动脉超声对缺血性脑血管病的诊断价值

中文摘要: 目的 分析经颅多普勒超声(TCD)联合颈动脉超声对缺血性脑血管病的诊断价值。方法 选取缺血性脑血管病患者93例,均行数字减影血管造影(DSA)、TCD及颈动脉超声检查,并以DSA结果为诊断标准,比较TCD和颈动脉超声单用及两者联合的检查结果,分析三者在缺血性脑血管病中的诊断灵敏度、特异度、阳性预测值及阴性预测值。结果 93例患者颅内外异常血管共763条。单用TCD示颅内血管异常73例,异常血管294条;单用颈动脉超声示颅外血管异常64例,异常血管275条;TCD联合颈动脉超声示颅内外血管异常89例,异常血管569条。TCD联合颈动脉超声在缺血性脑血管病诊断方面的灵敏度、特异度、阳性预测值及阴性预测值均高于单用TCD及单用颈动脉超声。结论 TCD联合颈动脉超声可提高缺血性脑血管病诊断的准确性,可作为颅内外血管病变诊断的有效检查手段。
Abstract:OBJECTIVE To analyze the diagnostic value of transcranial Doppler ultrasonography (TCD) combined with carotid artery ultrasound in ischemic cerebrovascular disease (ICVD). METHODS A total of 93 patients with ICVD were selected and treated with digital subtraction angiography (DSA), TCD and carotid artery ultrasound, with DSA results as the diagnostic criteria. The examination results of single TCD, single carotid artery ultrasound and TCD combined with carotid artery ultrasound were compared, and their sensitivity, specificity, positive predictive valve (PPV) and negative predictive value (NPV) in the diagnosis of ICVD were analyzed. RESULTS DSA showed 763 abnormal intra- and extra-cranial vessels in the patients, while single TCD revealed 73 abnormal intracranial vessels and 294 abnormal extracranial vessels, single carotid artery ultrasound indicated 64 abnormal intracranial vessels and 275 abnormal extracranial vessels, and TCD combined with carotid artery ultrasound suggested 89 abnormal intracranial vessels and 569 abnormal extracranial vessels in total. TCD combined with carotid artery ultrasound was evidently higher than single TCD and single carotid artery ultrasound in sensitivity, specificity, PPN and NPV in the diagnosis of ICVD. CONCLUSION TCD combined with carotid artery ultrasound can promote the accuracy in the diagnosis of ICVD, thus can be used as an effective examination method for the diagnosis of intra- and extra-cranial vascular diseases.
