微生态肠内营养对结直肠癌患者术后肠道菌群、免疫功能和并发症的影响

中文摘要: 目的 探讨微生态肠内营养对结直肠癌患者术后肠道菌群、细胞和体液免疫功能及并发症的影响。方法 将90例行结直肠癌根治术患者随机分为两组,每组各45例。观察组给予常规肠外营养,治疗组给予肠内营养联合微生态制剂。观察两组患者治疗前后肠道菌群、淋巴细胞亚群、免疫球蛋白和血清炎症因子的变化情况,并比较两组患者术后并发症的差异。结果 术前两组患者肠道菌群、淋巴细胞亚群、免疫球蛋白和血清炎症因子的差异均无统计学意义(P>0.05)。术后7 d,两组患者双歧杆菌、乳杆菌、双歧杆菌与肠杆菌比值(B/E)均较同组术前降低(P<0.05),大肠杆菌、肠球菌较术前升高(P<0.05),两组间的差异有统计学意义(P<0.05)。术后 7d,两组患者CD3+细胞、CD4+细胞、CD8+细胞、CD4+/CD8+、IgG、IgM和IgA均较同组术前下降(P<0.05),且观察组较治疗组下降更为明显(P<0.05)。术后7 d,两组患者CRP、TNF-α和IL-6水平均较同组术前升高(P<0.05),且观察组较治疗组升高更为明显(P<0.05)。治疗组术后并发症发生率为19.05%,观察组为23.26%,两组间差异无统计学意义(P>0.05)。结论 结直肠癌患者术后应用微生态肠内营养有助于纠正肠道菌群失衡,提高患者免疫功能,降低血清炎症因子水平,对术后并发症无明显影响。
Abstract:OBJECTIVE To investigate the effect of microbial enteral nutrition on intestinal microflora, cell and humoral immune function and complication in patients with colorectal cancer. METHODS 90 patients underwent radical resection of colorectal cancer were enrolled and randomly divided into2 groups, 45 cases in each group. The group A was given enteral nutrition combined with probiotics, while the group B was given routine parenteral nutrition. The changes of intestinal flora, lymphocyte subsets, immunoglobulin, serum inflammatory factors and postoperative complications were compared between the two groups. RESULTS There were no significant differences in intestinal flora, lymphocyte subsets, immunoglobulin and serum inflammatory factors before treatment (P>0.05). The ratios of Bifidobacterium, Lactobacillus, Bifidobacterium to Escherichia (B/E) were all decreased in the two groups after operation (P<0.05), Escherichia coli and Enterococcus increased (P<0.05), and the difference between the two groups was statistically significant P<(0.05). The expressions of CD3+ cells, CD4+ cells, CD8+ cells, CD4+/CD8+, IgG, IgM and IgA were all decreased in the two groups after operation (P<0.05), and lower in group B than in group A (P<0.05). The levels of CRP, TNF-α and IL-6 were all increased in the two groups after operation (P<0.05), and group B increased higher than those in group A (P<0.05). There was no significant difference in postoperative complications between the two groups (19.05%, 23.26%, respectively; P>0.05). CONCLUSION The application of microbial enteral nutrition in patients after colorectal cancer can help correct intestinal flora imbalance, improve immune function and reduce serum inflammatory factors, and it have no obvious effect on postoperative complications.
