中文摘要: 目的 探讨体外受精胚胎移植与自然受孕双胎妊娠并发症及新生儿围产结局，为双胎产前监护和临床管理提供理论依据。方法 回顾性分析540例体外受精胚胎移植产妇（IVF-ET组）和746例自然妊娠产妇（自然受孕组），比较两组产妇的一般情况、妊娠期并发症及新生儿围产结局。结果 （1）IVF-ET组产妇平均年龄大于自然受孕组（33.46±4.32岁 vs 31.19±4.49岁），差异有统计学意义（P﹤0.05），高龄产妇多见。（2）IVF-ET组产妇子痫前期、产后出血、前置胎盘、妊娠期糖尿病的发生率高于自然受孕组，差异有统计学意义（p<0.05）。（3）IVF-ET组双胎新生儿平均出生体重大于自然受孕组双胎（2387.01±511.79g vs 2287.00±478.99g）、孕周（35.33±2.39W vs 35.07±2.86W），差异有统计学意义（P﹤0.05）。IVF-ET组双胎新生儿围产结局与自然受孕组相当。结论 IVF-ET组产妇年龄偏大，高龄产妇多见；孕期并发症风险增加；但双胎新生儿不良结局并未增加。
Abstract:OBJECTIVE To investigate the maternal complications and pregnancy outcomes of twin pregnancy，and provide evidence for prenatal monitoring and clinical management of twin pregnancy.METHODS Clinical data were analyzed retrospectively of maternal general conditions,pregnancy complications and perinatal outcomes of 540 twin pregnancies conceived by vitro fertilization embryo transfer(IVF-ETgroup) and 746 spontaneous twin pregnancies(natural pregnancy group).Results (1)The average age of the IVF-ETgroup was significantly higher than that of natural pregnancy group (33.46±4.32 vs 31.19±4.49,P﹤0.05). Older mothers were more common.(2)The incidence of preeclampsia, postpIVF-ETum hemorrhage, placenata precia and gestational diabetes mellitus in the IVF-ETgroup was significantly higher than in the natural pregnancy group (P﹤0.05).(3)The avetage weight and gestational age of newborns in the IVF-ETgroup were significantly greater than in natural pregnancy group (2387.01±511.79g vs 2287.00±478.99g,35.33±2.39W vs 35.07±2.86W, P﹤0.05). The perinatal outcomes of IVF-ETgroup were similar to that of natural pregnancy group.CONCLUSIONS The women in IVF-ETgroup are uaually elderly.Older mothers were more common.They are more at risk of developing pregnancy complications.But the adverse perinatal outcomes don’t increase.
