中文摘要: 目的 基于剖宫产住院费用，探讨按疾病诊断相关分组(DRGs)方案的分组效果和病种分组规律，为按疾病诊断相关分组付费改革的推广提供参考依据。方法 回顾性收集2013年—2016年26 041例剖宫产病例病案首页信息，按照CN-DRGs方案进行分组，通过计算变异系数、方差减少量、相邻组相对变化和方差分析及多元统计分析评价分组效果。结果 总体入组率为91.21%，各组平均住院费用的CV值范围在0.2~0.4之间，RIV值为0.18。结论 建议增加“剖宫产伴随妇科手术”分组，征询更广大临床一线医生意见，进一步完善CN-DRGs分组方案。
Abstract:OBJECTIVE To provide reference for medical insurance DRGs reform, analysis of CN-DRGs scheme grouping effect and disease grouping rules based on cesarean hospitalization costs was conducted. METHODS A total number of 26 041 cases of hospital cesarean section from 2013 to 2016 was collected retrospectively and grouped by CN-DRGs program. The calculation of coefficient of variation, reduction in variance, relative change of adjacent groups, variance analysis and multivariate statistical analysis were used to evaluate the grouping effect. RESULTS The overall enrollment rate was 91.21%, the average coefficient of variation for each group ranged from 0.2 to 0.4, and the value of reduction in variance is 0.18. CONCLUSION We proposed to increase "cesarean section with gynecological surgery" group and recommend that drug.
