Abstract:OBJECTIVE To research the application situation of “Hospital-Community-Family” three-dimensional information management in the follow-up of hypertension in community. METHODS According to the random number table method, 140 cases of primary hypertension patients from the community were divided into the observation group and the control group. The control group was treated with the conventional model for community hypertension management. And the observation group was treated with Hospital-Community-Family” three-dimensional information management for community hypertension management. The control of blood pressure, the quality of life and the implementation of various compliance behavior were analyzed and analyzed in two groups. RESULTS After 6 months of implementation, the levels of SBP and DBP in the observation group were significantly lower than those in the control group, and all the SF-36 scores were significantly higher than those in the control group (P＜0.05). At the same time, the rates of compliance in the observation group were significantly higher than those of the control group after 6 months of implementation, and the differences were statistically significant (P＜0.05). CONCLUSION “Hospital-Community-Family” three-dimensional information management can significantly improve the self-management ability of hypertensive patients in the community. While the blood pressure can maintain a relatively stable blood pressure for a long time, and can also make them form a good living habit. This is of great significance for the prevention and treatment of hypertension in the community.
