中文摘要: 目的 以上海市嘉定区为例,对家庭医生目标薪酬模型及其应用开展研究。方法 构建嘉定区家庭医生责任目标年薪模型,并利用2017年第一季度数据进行模拟测算,对不同社区之间服务半径、人口等特殊情况,设立调整系数进行矫正。结果 嘉定区家庭医生平均签约率为8.75%,家庭医生与非家庭医生的平均薪酬比值为1.25;根据责任目标年薪模型测算出2017年第一季度家庭医生月薪在1.45万元~1.93万元之间,11家社区测算水平均高于实际发放水平,最高相差37.26%,2家社区低于实际发放水平,最低相差9.26%,差异主要由实际标化工作量完成率引起。讨论 实行基于标化工作量的家庭医生年薪制,可以在全区层面统一分配单价,整合社区平台功能,同时通过设置调整系数,体现不同区域的劳动成本,一定程度上实现了家庭医生多劳多得、优绩优酬。此外,目标薪酬制的实行应以良好的信息化平台为基础,同时保障政策平稳过渡。
Abstract:OBJECTIVE Take Jiading District as an example, to research on the application of the target annual salary system of family doctors; METHODS a target annual salary system was established and simulation calculation was applied using data in the 1st quarter of 2017. Adjustment coefficient was set according to service area and population structure of different community health centers; RESULTS the average contracting rate of family doctors in Jiading District was 8.75%. The salary ratio of family doctors and other working staff in community health centers was 1.25. According to the target salary model, the family doctors’ salaries in the 1stquarter of 2017 ranged from 14.5 to 19.3 thousand Yuan per month. The estimated numbers of 11 centers were higher than actual payment, with a maximum difference of 37.26%. While the estimated numbers of 2 centers were lower than actual payment, with a maximum difference of 9.26%. The difference is mainly caused by the completion rate of standardized working unit. DISCUSSION the implementation of a target salary model enables a unified allocation price of standardized working unit at district level and an integrated platform of communities. The establishment of adjustment coefficient reflects different labor costs in different centers and a fair allocation of remuneration according to working load can be achieved. The application of target annual salary model should be based on a well established information platform, and a smooth transition during the policy implementation should be ensured.
