中文摘要: 目的 分析典型案例中纵向整合措施对基层医疗机构服务质量的影响路径。方法 以民营医院托管公立基层医疗机构为典型案例，利用系统动力学分析质量影响路径和质量相关影响因素。结果 组织与管理、人员建设和服务能力等三个关键层面的整合举措对基层医疗机构服务质量产生的影响较大。对公共卫生服务质量直接影响因素较少，但临床服务质量的改善会侧面促进其提升。利益分配相关要素对质量影响较小。结论 在巩固组织与管理、人员建设和服务能力等层面整合措施效果的基础上，应进一步加大对利益分配机制相关整合举措的实施并监测评价，以进一步提升基层医疗机构服务质量。
Abstract:OBJECTIVE To analyze the influencing path of vertical integrating measures on the quality of primary health institution in the case study. METHODS The integration of primary health institution entrusted to private hospital was used as the typical case. The influencing path and quality related factors were analyzed by means of systematic dynamics.RESULTS Specific measures at organization and management, human resources and service capacity produced more important effects on quality of primary health institution. The direct influence on the quality of public health was limited while the improvement of clinical service quality could promote its quality.CONCLUSION Specific measures on interest allocation mechanism and monitoring and evaluation should be emphasized to further improve the quality of primary health institution based on the consolidation of impact resulted from organization and management, human resources and service capacity.
