中文摘要: 目的 探究视频脑电图与剥夺睡眠脑电图在小儿癫痫诊断中的对比。方法 回顾性分析近年来郑州大学附属儿童医院收治的疑似癫痫患儿204例，均分别进行视频脑电图、剥夺睡眠脑电图检测，对比两种检测方法的诊断结果。结果 视频脑电图组检出敏感度、特异度优于剥夺睡眠脑电图组。视频脑电图组癫痫放电检出率为62.5%，高于剥夺睡眠脑电图组的52.0%。剥夺睡眠脑电图组对1~3岁患儿癫痫放电检出率高于其他年龄段；而视频脑电图组各年龄段检出率比较无显著性差异。剥夺睡眠脑电图组睡眠期癫痫样放电率明显高于其他时期；视频脑电图组在各时期癫痫样放电率比较无显著性差异。结论 视频脑电图对小儿癫痫检出率较高，且其诊断敏感度、特异度较高，但对于1~3岁小儿建议采用剥夺睡眠脑电图检查。
Abstract:Objective to explore the comparative analysis of video EEG and sleep deprived EEG in the diagnosis of infantile epilepsy. Methods A retrospective analysis of 204 children with suspected epilepsy in our hospital in recent years were performed with video electroencephalogram (EEG) and sleep deprivation electroencephalogram (EEG), and the results of the two methods were compared. Results The sensitivity and specificity of the video EEG group were better than those of the sleep-deprived EEG group . The detection rate of epileptic discharge in the video EEG group was 62.5%, which was higher than 52.0% in the sleep-deprived EEG group . The detection rate of epilepsy discharge in children with 1～3 years of age in the sleep-deprived EEG group was higher than that in other age groups , but there was no significant difference in the rate of detection between age groups in the video EEG group . The rate of epilepsy-like discharge in the sleep-evoked EEG group was significantly higher than that in other periods ; there was no significant difference in the rate of epileptic-like discharge in the video EEG group .Conclusion Video EEG has a higher detection rate of epilepsy in children, and its diagnostic sensitivity and specificity are higher. However, sleep deprivation electroencephalogram examination is recommended for children aged 1 to 3 years.
