老年人ACR检测及其与糖尿病、高血压肾功能损害的相关性研究

中文摘要: 目的 研究老年人尿微量白蛋白/尿肌酐比值(urinary microalbumin/ creatinine ratio,ACR)检测及其与糖尿病、高血压肾功能损害的相关性。方法 选择2017年6月—2018年6月在上海市嘉定工业区社区卫生服务中心接受治疗的118例符合条件的患者作为本次的研究对象。按照患者的基础疾病类型对患者进行分组,分别为高血压组、糖尿病组以及高血压合并糖尿病组, 再选择我中心老年人体检健康50例作为本次的对照组。对所有研究对象的血肌酐(SCr)、尿素氮(BUN)、尿酸(UA)、尿微量白蛋白(urine microalbuminuria,UmALB)以及ACR等生化指标水平进行比较与分析。结果 对照组与高血压组、糖尿病组以及高血压+糖尿病组的研究对象的肾损害生化指标UA、ACR以及UmAlb的阳性率相比, 数据存在明显差异(P<0.05)。分级不同的高血压患者的ACR阳性率存在明显差异(P<0.05);高血压2级患者与高血压1级、3级的患者相比,数据无明显差异(P>0.05);高血3级患者ACR阳性率与高血压1级患者的ACR阳性率比较, 数据存在明显差异(P<0.05)。不同病程的高血压、糖尿病患者间ACR阳性率无明显差异(P>0.05)。结论 ACR能够作为早期诊断高血压、糖尿病患者肾脏损伤情况的指标,有着较高的敏感性。
Abstract:Objective To study the relationship between ACR detection and renal function impairment in elderly patients with diabetes mellitus and hypertension. Methods 118 cases eligible patients treated in target hospital from June 2017 to June 2018 were selected as the subjects of this study and divided into hypertension group, diabetes group and hypertension combined with diabetes group according to their basic disease types. 50 healthy people in target hospital were selected as the control group. The levels of serum creatinine, urea nitrogen , uric acid, UmAlb and ACR were compared and analyzed. Results The positive rates of UA, UmAlb and ACR in hypertensive group, diabetic group and hypertensive + diabetic group were significantly higher than control group (P<0.05). There was significant difference in ACR positive rate between different grades of hypertension subgroups (P<0.05). There was no significant difference between grade 2 and grade 1 and grade 3 (P>0.05). There was significant difference between grade 3 and grade 1 ACR positive rate (P<0.05). There was no significant difference in ACR positive rate between different course subgroups of hypertension and diabetes mellitus (P>0.05). Conclusion Hypertension, hypertension and diabetes often accelerate the occurrence and development of kidney damage. ACR had great significance in the diagnosis of early renal damage in hypertension and diabetes mellitus, and could be used as a sensitive index in the diagnosis of early renal damage in hypertension and diabetes mellitus.
