超声生物物理评分及胎儿脐动脉血S/D检测在胎儿宫内窘迫诊断中的应用价值

中文摘要: 目的 分析超声生物物理评分及胎儿脐动脉血收缩期与舒张期流速比(S/D)检测在胎儿宫内窘迫诊断中的应用,为临床诊断提供指导。方法 2015年1月—2018年12月期间,将我院收治的70例孕妇作为研究对象。采用胎儿电子监护仪、多普勒超声诊断仪测定胎儿生物物理评分(BPS)及脐动脉血S/D。观察孕妇BPS评分和S/D比值分别与新生儿Apgar≤7与羊水污染的关系及BPS评分与S/D值的关系。结果 在BPS>6组中,羊水污染Ⅱ度(3.17%)、Ⅲ度(3.17%)和新生儿Apgar≤7(1.59%)明显低于BPS≤6组的(28.57%)、(42.86%)和(14.29%),而总不良率(6.35%)显著低于BPS≤6组的(85.71%),两组比较具有统计学意义(P<0.05)。在S/D<3组中,羊水污染Ⅱ度(3.13%)、Ⅲ度(4.69%)和新生儿Apgar≤7(3.13%)明显低于S/D≥3组的(33.33%)、(16.67%)和(16.67%),而总不良率(10.93%)显著低于S/D≥3组的(66.67%),两组比较具有统计学意义(P<0.05)。在BPS>6组中,S/D<3占比(85.71%)明显高于BPS≤6组中的(14.29%),而S/D≥3占比(3.17%)明显低于BPS≤6组中的(42.86%),两组比较具有统计学意义(P<0.05)。结论 胎儿BPS评分和脐动脉血S/D对孕妇胎儿宫内窘迫的诊断具有重要的临床意义,可明显降低胎儿死亡率,在临床中值得推广应用。
Abstract:OBJECTIVE To study application value in intrauterine fetal distress by ultrasound biophysical score and fetal umbilical artery blood S/D detection, so as to provide guidance for clinical diagnosis. METHODS 70 cases pregnant women were served as objects of study from January 2015 to December 2018. Fetal biophysical score (BPS) and umbilical artery blood S/D were measured by fetal electronic monitor and Doppler ultrasound diagnostic instrument. The relationship between BPS score and S/D ratio and neonatal Apgar<7 and amniotic fluid pollution, and the relationship between BPS score and S/D value were observed, respectively. RESULTS Amniotic fluid pollution degree II (3.17%) degree Ⅲ (3.17%) and Apgar≤7 (1.59%) in BPS>6 groups were statistically lower than (28.57%) (42.86%) and (14.29%) in BPS≤6 groups, while total adverse rate (6.35%) was statistically lower than (85.71%) in BPS≤6 groups, the difference was statistically significant (P<0.05). Amniotic fluid pollution degree II (3.13%) degree Ⅲ (4.69%) and Apgar≤7 (3.13%) in S/D<3 groups were statistically lower than (33.33%) (16.67%) and (16.67%) in S/D≥3 groups, while total adverse rate (10.93%) was statistically lower than (66.67%) in S/D≥3 groups, the difference was statistically significant (P<0.05). S/D<3 (85.71%) in BPS>6 groups were statistically rather than 14.29% in BPS≤6 groups, while S/D≥3 (3.17%) was statistically lower than (42.86%) in BPS≤6 groups, the difference was statistically significant (P<0.05). CONCLUSION Fetal BPS score and umbilical artery blood S/D had important clinical significance in the diagnosis of fetal distress in pregnant women, and could significantly reduce fetal mortality, which was worthy of promotion and application in clinical practice.
