中文摘要: 目的 探讨缓解期抑郁症患者残留症状对功能损伤的影响。方法 选择2017年1月—2018年12月上海市宝山区精神卫生中心门诊收治的62例抑郁症患者,所有患者急性期治疗后达到临床缓解。根据席汉残疾量表(SDS)分为功能正常组(n=40)和功能异常组(n=22),比较两组残留症状等相关指标,分析残留症状与功能损伤的相关性,并采用多因素logistics回归分析残留症状对功能损伤的影响。结果 睡眠紊乱与SDS总分、工作/学习功能呈显著正相关,SDS总分及各维度与精神运动性变化、沮丧/悲伤等残留症状呈显著正相关(P﹤0.05);与功能正常组比较,功能损伤组首次抑郁发病、合并躯体疾病、躯体症状程度及残留症状比例显著增加,本次服药时间明显缩短(P﹤0.05);多因素logistic回归结果显示,躯体症状程度、残留症状是缓解期抑郁症患者功能损伤的独立危险因素,而首次抑郁发病则是保护性因素(P﹤0.05)。结论 缓解期抑郁症患者可出现不同程度的功能损伤,与睡眠紊乱、精神运动性变化、沮丧/悲伤等残留症状密切相关,残留症状是功能损伤发生的影响因素。
Abstract:OBJECTIVE To investigate the effects of residual symptoms on functional impairment in patients with remission depression. METHODS 62 patients with depression admitted to the outpatient department of target hospital from January 2017 to December 2018 were selected, and all patients turned clinical remission after acute stage treatment. According to the Sheehan Disability Scale(SDS), the patients were divided into the normal functional group (n=40) and the abnormal functional group (n=22), and the residual symptoms and other related indicators of these two groups were compared, the correlation between residual symptoms and functional injury were analyzed. The effects of residual symptoms on functional injury were analyzed by multiple Logistics regression. RESULTS Sleep disorders were positively correlated with SDS score, work/learning function, and SDS score and each dimension were positively correlated with mental activity changes, depression/sadness and other residual symptoms (P<0.05). Compared with the normal functional group, the first onset of depression, the comorbidity of somatic diseases, the degree of somatic symptoms and the proportion of residual symptoms were significantly increased in the abnormal functional group, and the duration of medication was significantly shortened (P<0.05). Multiple Logistic regression showed that degree of somatic symptoms and residual symptoms were independent risk factors for functional impairment in patients with remission depression, while the onset of primary depression was a protective factor(P<0.05). CONCLUSION Patients with remission depression might suffer functional impairment of varying degrees, which was closely related to residual symptoms such as sleep disorders, changes in mental activity, depression/sadness. Residual symptoms were the influencing factors of functional impairment.
