不同定位技术在触诊阴性乳腺病灶中的应用探讨

中文摘要: 目的 探讨超声、钼靶X线和乳管镜三种不同定位技术在触诊阴性乳腺病灶(NPBL)中的应用价值。方法 回顾性分析2016年5月—2019年5月甘肃省妇幼保健院收治的100例NPBL患者临床资料,56例行超声体表定位切除,21例行钼靶X线导丝定位切除,23例伴乳头溢液症状者行乳管镜导丝定位切除,根据患者病理结果,分析三种定位技术对NPBL的诊治价值。结果 56例经超声体表定位患者,联合美兰染色后可准确定位并切除病灶,术后病理学检查均证实存在病变,其中恶性病变20例,良性病变36例,病理结果与BIRADS 分级一致;21例钼靶检查患者,均可见钙化病变,联合导丝定位后可准确切除病灶,术后证实4例恶性病变,17例良性病变; 23例乳管镜检查均见乳腺导管内病变, 且与术后病理诊断符合率为91.3%,经导丝定位联合美兰染色,可准确定位并切除病灶。结论 超声、钼靶和乳管镜三组定位技术在NPBL患者定位、定性中均有较高的应用价值,能准确进行病灶定位、切除,提高了NPBL诊治水平。

Abstract:Objective To explore the application value of 3 different localization techniques of ultrasound, mammography and ductoscopy in nonpalpable breast lesions (NPBL). Methods The clinical data of 100 patients with NPBL admitted to the hospital from May 2016 to May 2019 were retrospectively analyzed. 56 patients underwent ultrasound body surface localization and resection, and 21 patients underwent mammography guidewire localization and resection, and 23 patients with nipple discharge were treated with ductoscopy guidewire localization and resection. According to the pathological results of the patients, the value of the three localization techniques in the diagnosis and treatment of NPBL was analyzed. Results In 56 patients with ultrasound body surface localization, lesions could be accurately located and removed after combined with methylene blue staining, and postoperative pathological examination confirmed the presence of lesions, including 20 malignant lesions and 36 benign lesions. And the pathological results were consistent with BIRADS classification. In 21 patients with mammography examination, calcified lesions were seen, and the lesions could be accurately removed after combined guidewire localization, and 4 cases of malignant lesions and 17 cases of benign lesions were confirmed. In 23 cases with ductoscopy, lesions were found in breast duct, and its coincidence rate of pathological diagnosis was 91.3%, and lesions were accurately located and removed after guidewire localization combined with methylene blue staining. Conclusion These localization techniques of ultrasound, mammography and ductoscopy had high application value in the localization and qualitation of NPBL patients. It could accurately locate and excise lesions and improve the diagnosis and treatment of NPBL level.
