不同级别医疗机构门诊患者对分级诊疗的认知差异探讨

中文摘要: 目的 了解不同级别医疗机构门诊患者对分级诊疗的认知差异,为构建分级诊疗制度和完善医疗服务体系提供参考依据。方法 自行设计“患者分级诊疗认知情况调查表”,对自贡市一、二和三级医疗机构的300名门诊患者进行问卷调查。结果 受调查患者对分级诊疗的知晓率为46.67%,赞成率为59.00%,有40.33%的患者接受过分级诊疗的宣传。不同级别医院的患者对分级诊疗的知晓率、赞成率、可行度、双向转诊经历和常见病多发病的首选医院等问题的认知情况差异具有统计学差异(P<0.05)。结论 各级医疗机构均应加大对分级诊疗制度的宣传力度,提升基层医疗机构的医疗水平和服务能力,进一步完善双向转诊、上下联动的分级诊疗机制,促进分级诊疗制度的深入推进。

Abstract:OBJECTIVE To understand the cognitive differences of hierarchical diagnosis among outpatients in different levels of medical institutions, and to provide references for the establishment of hierarchical diagnosis system and the improvement of medical service system. METHODS The self-designed “questionnaire on patients’ cognition of hierarchical diagnosis” was used to conduct a questionnaire survey among 300 outpatients from the first, second and third-level medical institutions in Zigong. RESULTS The awareness rate of patients surveyed for graded diagnosis and treatment was 46.67%, the approval rate was 59%, and 40.33% patients had received the publicity of graded diagnosis and treatment. The difference of patients’ cognition of graded diagnosis was statistically significant (P < 0.05). CONCLUSION Medical institutions at all levels should strengthen the publicity of the hierarchical diagnosis and treatment system, improve the medical level and service capacity of grassroots medical institutions, further improve the two-way referral and the linkage of the hierarchical diagnosis and treatment mechanism, and promote the in-depth promotion of the hierarchical diagnosis system.
