中文摘要: 目的 探讨社区—家庭心理干预对脑卒中后抑郁患者情绪及康复效果的临床作用。方法选取2017年12月—2018年12月上海市黄浦区南京东路街道社区卫生服务中心内科收治的住院患者195例脑卒中患者，按照随机对照法随机分为社区心理干预组(n=65)、家庭心理干预组(n=65)及联合组(n=65)，三组患者均在常规康复护理的基础上实施心理干预，社区心理干预组施行社区医院内心理干预，家庭心理干预组施行出院后家庭心理干预，联合组采用医院内心理干预联合出院后家庭心理干预。比较干预前、干预3个月后和干预6个月后三组患者的负性情绪和康复效果。结果 与干预前比较，干预3个月后和干预6个月后三组患者NIHSS评分显著降低(P＜0.05)，干预3个月后和干预6个月后联合组NIHSS评分显著低于其他两组(P＜0.05)；与干预前比较，干预3个月后和干预6个月后三组患者Fugl-meyer评分显著升高(P＜0.05)，干预3个月后和干预6个月后联合组Fugl-meyer评分显著高于其他两组(P＜0.05)；与干预前比较，干预3个月后和干预6个月后三组患者SAS和SDS评分显著降低(P＜0.05)，干预3个月后和干预6个月后联合组SAS和SDS评分显著低于其他两组(P＜0.05)。结论 社区—家庭心理干预可促进脑卒中患者神经功能的恢复，改善运动功能，降低患者的负性情绪，促进患者康复。
Abstract:OBJECTIVE To investigate the quality of sleep in elderly hypertensive patients with insomnia, and to explore the influencing factors. METHODS Using the convenient sampling method, the elderly hypertensive patients with insomnia who were treated in the Department of Cardiology, People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine from March to June 2019 were selected as subjects. They were investigated by general data metering and Pittsburgh Sleep Quality Index Rating Scale. RESULTS The total score of sleep quality in elderly hypertensive patients with insomnia was(12.30±2.65). The results of multiple linear regression analysis showed that gender, education level, duration of hypertension, high blood pressure and hypertension were the influencing factors of total sleep quality scores. Explain 58.40% of the variation. CONCLUSION The scores of total sleep quality of elderly hypertensive patients with insomnia are higher, closely related to patients’ blood pressure control. Nursing staff should pay attention to the blood pressure grading and control of such patients, and carry out personalized nursing and health guidance for patients of different genders and education levels. Help patients improve disease self-management skills, lifestyle and sleep habits, to improve the quality of sleep in patients.
