三维盆底超声在不同BMI及孕周产妇盆底功能 评价中的价值分析

中文摘要: 目的:探讨三维盆底超声对不同BMI及孕周产妇进行盆底功能评价的临床价值。方法:以2019年1月—2020年 1 月在天水市第一人民医院产科接受初次产前检查的 132 名孕妇 (孕周≤8 周) 为研究对象,依据 BMI 指数分为 3 组:A 组 (BMI<16.85 kg/m2) 42 人,B 组 (16.85 kg/m2≤BMI≤23.81 kg/m2) 48 人,C 组 (BMI>23.81 kg/ m2) 42 人,分别于孕早期、 孕中期及孕晚期对3组孕妇开展三维盆底超声检查,评估产妇盆底功能。结果:A组孕妇孕中期静息、缩肛、Valsalva动作时 肛提肌裂孔面积,早期尿道旋转角度,膀胱颈下移距离及逼尿肌厚度均高于孕早期,差异有统计学意义 (t值分别为2.180、 5.867、2.943、8.531、11.896、9.871,P 值均<0.05),孕晚期上述测量指标均高于孕早期,差异有统计学意义 (t值分别为 4.761、7.566、4.799、11.866、15.485、13.559,P值均<0.05);B组孕中期上述测量指标均高于孕早期,差异有统计学意义 (t值分别为2.070、5.501、2.922、9.395、11.820、9.959,P值均<0.05),孕晚期上述测量指标均高于孕早期,差异有统计学 意义 (t 值分别为 4.429、7.621、5.966、13.483、16.042、11.595,P 值均<0.05);C 组孕中期上述测量指标均高于孕早期, 差异有统计学意义 (t值分别为 4.639、6.834、5.353、10.272、13.347、13.610,P值均<0.05),孕晚期上述测量指标均高于 孕早期,差异有统计学意义 (t值分别为 6.445、9.019、7.723、14.820、18.175、14.146,P值均<0.05)。结论:三维盆底超 声应用于评估不同BMI及孕周产妇盆底功能临床价值较高。

Abstract:OBJECTIVE To explore the clinical value of three-dimensional pelvic floor ultrasound applied to the evaluation of pelvic floor function of women with different BMI and gestation. METHODS The subjects were 132 pregnant women who underwent the initial obstetrical examination in the obstetrics department from January 2019 to January 2020 (pregnancy week≤8 weeks) and were divided into 3 groups according to the BMI index, 42 cases(BMI<16.85 kg/m2 ) in group A and 48 cases(16.85kg/m2≤BMI≤ 23.81kg/m2 ) in group B, 42 cases(BMI>23.81kg/m2 ) in group C. Three-dimensional pelvic floor ultrasound examination was carried out in three groups in the first, second and third trimesters of pregnancy to evaluate maternal pelvic floor function. RESULTS Rift ar⁃ ea of the anal lift muscle in resting, contractility and Valsalva movement, early urethral rotation angle, bladder neck downward shift distance and detrusor thickness comparison in the A group were higher than in the first trimester, and the difference was statistically significant (t= 2.180, 5.867, 2.943, 8.531, 11.896, 9.871, P<0.05). The above measures in higher stages of pregnancy than in early pregnancy, and the difference was statistically significant (t=4.761, 7.566, 4.799, 11.866, 15.485, 13.559, P<0.05). The above mea⁃ sures were higher in the B group than in early pregnancy, and the difference was statistically significant (t=2.070, 5.501, 2.922, 9.395, 11.820, 9.959, P<0.05). The above measures were higher than in late pregnancy, and the difference was statistically signifi⁃ cant (t=4.429, 7.621, 5.966, 13.483, 16.042, 11.595, P<0.05). C group were higher than in the early trimester, and the difference was statistically significant (t=4.639, 6.834, 5.353, 10.272, 13.347, 13.610, P<0.05). The above measures were higher than in late pregnancy, and the difference was statistically significant (t=6.445, 9.019, 7.723, 14.820, 18.175, 14.146, P<0.05). CONCLUSION Three-dimensional pelvic floor ultrasound was of high clinical value in evaluating pelvic floor function of women with different BMI and gestational age.
