中文摘要: 目的 了解低出生体重儿早期(3月龄内)神经精神发育情况。方法 2016年12月-2017年4月出生纳入闵行区儿童保健管理系统的早产、足月低出生体重儿,在其3月龄内完成Gesell发育量表评估,共728人。结果 胎龄<37周两组在早期未矫正胎龄评估中各能区DQ均值均低于75分,正常足月组各能区DQ分值介于85~91分之间。胎龄<34周组各功能区DQ分值没有差异,正常足月组各功能区DQ分值差异显著。胎龄<37周两组,校正胎龄前DQ分值≤75分,占91.18%,校正胎龄后DQ分值≤75分,占13.03%。三组适应性DQ分值的在各自五个功能区的排位顺序显示34周≤胎龄<37周组高于正常足月组高于胎龄<34周组。结论 基于早期干预的早期评估,未校正胎龄的评估结果更能直观了解婴儿早期发育现状。对于>34周低出生体重儿早期发育评估能较早发现其适应性发育和各能区发育均衡性的问题。

Abstract:OBJECTIVE To understand the neurodevelopment of low birth weight(LBW) infants under 3 months old.METHODS A cohort of 728 preterm and LBW infants, who were born in December 2016 to April 2017 and included in children’s health management system of Minhang district, were assessed with Gesell Developmental Schedules at 0-3 months of age.RESULTS The Development Quotient score(DQs) of preterm LBW assessed at uncorrected gestational age(GA) were all less than 75. And those of full term LBW infants were among 85-91. In the LBW infants with GA<34weeks , there was no difference between the DQs of five functional areas . However, in the full term LBW infants, the difference was significant. 91.18% of preterm LBW infants assessed at uncorrected GA had more than one DQs less than 75, however 13.03% of them assessed at corrected GA showed up. The DQs of the adaptability in the LBW infants with GA of 34-37weeks were higher than those in full term LBW infants, who has higher DQs of the adaptability than LBW infants with GA<34weeks. CONCLUSION The results of the early assessment taken at uncorrected GA are more intuitive to understand the early neurodevelopment of LBW infants. For LBW infants with GA>34weeks, the problem of development delay in adaptability and imbalance development in five functional areas can be identified by early neurodevelopmental assessment.
