中文摘要: 目的 探讨检测血清胱抑素(Cys-C)、糖化血红蛋白(HbA1c)、超敏C反应蛋白(Hs-CRP)及尿微量清蛋白(U-mALB)在2型糖尿病早期肾损伤中的诊断价值。方法 选取复旦大学附属中山医院青浦分院2016年1月—2017年12月94例疑似2型糖尿病早期肾损伤患者作为本次研究对象,根据CT等检查结果将患者分为对照组(2型糖尿病30例)与观察组(2型糖尿病早期肾损伤64例)。分别检测两组血清Cys-C、HbA1c、Hs-CRP和U-mALB,并比较各指标单独检测、联合检测敏感度、准确度及特异度。结果 观察组患者血清Cys-C、Hs-CRP及U-mALB水平明显高于对照组,差异有统计学意义(P<0.05);两组血清HbA1c水平比较,,差异无统计学意义(P>0.05)。单纯检测各指标时,其中检测U-mALB阳性率最高,而检测Hs-CRP的阳性率最低,联合检测血清Cys-C、HbA1c、Hs-CRP及U-mALB阳性率明显高于单纯检测单一指标检测阳性率。联合检测血清Cys-C、HbA1c、Hs-CRP及U-mALB敏感度、准确度及特异度明显高于单纯检测单一指标。结论 2型糖尿病早期肾损伤患者血清Cys-C、Hs-CRP)及U-mALB明显升高,且联合检测Cys-C、HbA1c、Hs-CRP及U-mALB可提高检测准确度,为临床疾病诊断和治疗方案的制定提供有力依据。
Abstract:objective To explore the diagnostic value of serum Cystatin (Cys-C), HbA1c, Hs-CRP and U-mALB in early renal injury in type 2 diabetes.Methods From January 2016 to December 2017, 94 patients with suspected early type 2 diabetes mellitus were selected as the subjects of this study. Patients were divided into control group (type 2 diabetes, 30 cases) and observation group according to CT and other test results. Type 2 diabetes early kidney injury, 64 cases). The serum levels of cystatin (Cys-C), glycosylated hemoglobin (HbA1c), high-sensitivity C-reactive protein (Hs-CRP), and urinary microalbumin (U-mALB) were detected and compared. However, the positive rates of combined detection of serum Cys-C, HbA1c, Hs-CRP, and U-mALB were significantly higher than those of single detection. The positive rate of index detection was . The sensitivity, accuracy and specificity of combined detection of serum Cys-C, HbA1c, Hs-CRP, and U-mALB were significantly higher than those of single detection alone, Conclusion Serum cystatin (Cys-C), high-sensitivity C-reactive protein (Hs-CRP), and urinary microalbumin (U-mALB) were significantly elevated in patients with early-stage renal impairment in type 2 diabetes mellitus, and combined detection of Cys-C, HbA1c, Hs-CRP and U-mALB can improve the detection accuracy, and provide a strong basis for clinical diagnosis and treatment plan.
