肺表面活性物质联合持续气道正压通气治疗新生儿呼吸窘迫综合征临床观察

中文摘要: 目的 探讨持续气道正压通气(CPAP)联合肺表面活性物质(PS)在新生儿呼吸窘迫综合征(NRDS)中的应用效果。方法 选取2016年4月—2017年12月我院收治的NRDS患儿100例,按随机数字表法分为对照组和观察组,每组各50例。对照组给予PS治疗,观察组给予PS联合CPAP治疗,对比两组患儿住院天数、血气分析指标及并发症发生情况。结果 两组治疗前pH值、二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)和氧合指数(PaO2/FiO2)比较,差异无统计学意义(P>0.05);观察组治疗后pH值、PaCO2低于对照组,PaO2、PaO2/FiO2高于对照组,差异有统计学意义(P<0.05);两组视网膜病、气胸及坏死性小肠结肠炎发生率比较,差异无统计学意义(P>0.05);观察组支气管肺发育不良发生率、住院时间均优于对照组,差异有统计学意义(P<0.05)。结论 CPAP联合PS治疗NRDS可以有效改善患儿血气指标,减少支气管发育不良的发生,具有临床推广应用价值。

Abstract:OBJECTIVE To investigate the effect of continuous positive airway pressure (CPAP) combined with pulmonary surfactant (PS) in neonatal respiratory distress syndrome (NRDS). METHODS A total of 100 children with NRDS admitted to our hospital from April 2016 to December 2017 were enrolled. The patients were divided into the control group and the observation group according to the random number table, with 50 cases in each group. The control group was treated with PS, and the observation group was given PS combined with CPAP. The hospitalization days, blood gas analysis indexes and complications were compared between the two groups. RESULTS The pre-treatment pH, carbon dioxide partial pressure (PaCO2), arterial oxygen partial pressure (PaO2), oxygenation index (PaO2/FiO2), the difference was not statistically significant. Significance (P>0.05); pH value and PaCO2 in the observation group were lower than those in the control group, PaO2 and PaO2/FiO2 were higher than the control group, the difference was statistically significant (P< 0.05); There was no significant difference in the incidence of retinopathy, pneumothorax and necrotizing enterocolitis between the two groups (P>0.05). The incidence of bronchopulmonary dysplasia and hospitalization time in the observation group were better than the control group. Academic significance (P<0.05). Conclusion CPAP combined with PS in the treatment of NRDS can effectively improve the blood gas index and reduce the occurrence of bronchial dysplasia. It has clinical application value.
