中文摘要: 目的 探讨术前糖负荷对围手术期应激性高血糖患者血糖和HOMA指数的影响。方法 将手术患者分为研究组和对照组。研究组术前3 h给予糖负荷,对照组给予常规禁食8~12 h。测定两组术前、术后1天h和术后3天的血糖和HOMA指数。结果 (1)两组患者术后的血糖值及胰岛素均高于术前血糖值, 术后研究组患者的血糖值及胰岛素低于对照组(P均>0.05)。(2)研究组术后第1 d和术后第3 d的HOMA-IS均高于对照组(P均<0.05)。与术前相比,两组患者术后第1 d和第3 d的HOMA-ISI均升高(P均<0.05);研究组术后第1 d和术后第3 d的HOMA-IS均高于对照组(P均<0.05)。与术前相比,两组患者术后第1 d和第3 d的HOMA-IR均降低(P均<0.05);研究组术后第1 d的 HOMA-IR低于对照组(P<0.05)。结论 术前糖负荷可以减少围手术期应激性高血糖的发生率。
Abstract:Objective To investigate the effect of preoperative sugar load on blood glucose and Homa index in patients with stress hyperglycemia during perioperative period. Methods The surgical patients were divided into study Group and control group. The sugar load was given in the Study group before Operation 3h, and the control group was given routine fasting 8~12h. The blood glucose and Homa index of two groups were measured 1 days before and after operation, 3 days after operation. Results The blood glucose value and insulin were higher than the preoperative blood glucose value in the two groups of (1) and the blood glucose value and insulin were lower in the postoperative study group than in the control group (both p>0.05). In the. (2)Study Group, the Homa-is was higher than that of the control group (p<0.05) after Operation 1 d and postoperative 3 d. Compared with before operation, the Homa-isi of 1d and 3 d were increased (both p<0.05) after operation in both groups, and the homa-is of 1 d and postoperative 3 d were higher in the study group than in the control group (both p<0.05). Compared with preoperative, the homa-ir of 1d and 3d decreased (both P<0.05) after operation in both groups, and the homa-ir of 1d after operation in the study group was lower than that of the control group (P<0.05). Conclusion preoperative sugar load can reduce the incidence of stress hyperglycemia during perioperative period.
