痴呆相关预防知识的健康教育对轻度认知功能障碍人群的干预效果研究

中文摘要: 目的 调查上海市浦东新区入册轻度认知功能障碍(mild cognitive impairment,MCI)老人痴呆相关知识知晓情况及共病高血压糖尿病对健康教育效果的影响。方法 采用自行设计的老年痴呆防治相关知识知晓率的调查表,对经过一年科普教育且2016年建档的942名MCI干预项目老人开展调查,根据有无高血压(Hypertension,HT)、糖尿病(diabetes mellitus,DM)或者是否遵医嘱用药,是否有运动习惯等将MCI分组,比较各组的知晓率差异。结果 社区MCI老人对痴呆概念的知晓率最高,其次为危险因素,危害性和就医信息。共病糖尿病MCI老人的知晓率高于高血压者,用药组高于不用药组,且运动组高于不运动组,差异均有统计学意义(P﹤0.05或P﹤0.01)。回归分析显示,痴呆知晓率与年龄负相关,与运动正相关,差异有统计学意义(P﹤0.01)。结论 现阶段以提高人群的MCI知识知晓率为主要抓手,健康教育对预防痴呆具有积极意义;年龄较小、有运动习惯者对MCI健康教育接受程度较高。未来研究需要增加睡眠、饮食因素的调查,进一步明确健康教育对MCI者健康生活方式的影响。
Abstract:OBJECTIVE To investigate the knowledge of the aged who were recorded in Shanghai Pudong New Area and the impact of the diabetic mellitus and hypertension on the awareness of the related knowledge of dementia after education. METHODS A self-designed questionnaire was used to investigate the knowledge rate of Alzheimer’s disease in 942 recorded MCI in 2016. All the participants were divided and compared the awareness of knowledge according to the chronic disease, drug compliance and physical exercise. RESULTS The community MCI participants had the highest awareness of the concept of dementia, followed by risk factors, hazards and medical information. The rates of the awareness of the total scores, risk factors of dementia and medical treatment information, were higher in MCI+DM group than those of MCI+HB group.The awareness of the drug-compliance group were higher than those in the no compliance group, while that of the regular physical exercise group were higher than that of no regular physical exercise group, and the differences were statistically significant(P<0.05 or P<0.01). The regression analysis showed that the awareness rate of dementia was negatively correlated with age, and positively related to physical exercise, and the difference was statistically significant (P<0.01). CONCLUSION The concept of dementia-related knowledge was the highest, followed by the risk factors, the hazard, and the medical information. That meant that the community health education work needed to be further strengthened in order to achieve early diagnosis and intervention.
