中文摘要: 目的 构建适用于社区的糖尿病风险评估体系。方法 采取文献回顾和专家咨询，先，提取国内外常见的糖尿病风险评估模型中的相关指标与测量原则，进行初步筛选。后选取30名社区卫生服务中心从事糖尿病慢病管理相关岗位的专家，通过２轮Delphi专家咨询，初步构建出社区糖尿病风险评估模型。结果 共选取了14个符合标准的国内外糖尿病风险评估模型，最终得到4大类34项风险评估指标。经过２轮专家咨询最终形成了4大类（基本信息类、个人史类、生活方式类、生化指标类）29个指标的社区糖尿病风险评估体系初始方案。结论 包括4大类29个指标的社区糖尿病风险评估体系初始方案可为最终社区糖尿病综合管控模式的构建提供参考。
Abstract:OBJECTIVE To construct a diabetes risk assessment system for the community. METHODS Through literature review and expert consultation, it firstly extract and conducted preliminary screening relevant indicators and measurement principles in the common diabetes risk assessment models at home and abroad. After the selection of 30 community health service centers engaged in diabetes chronic disease management related posts, through two rounds of Delphi expert consultation, initially a community diabetes risk assessment model was constructed. RESULTS A total of 14 domestic and international diabetes risk assessment models were selected, and finally 34 risk assessment indicators were obtained in 4 categories. After two rounds of expert consultation, the initial plan of the community diabetes risk assessment system with 29 indicators (basic information, personal history, lifestyle, biochemical indicators) was formed. CONCLUSION Including 4 categories and 29 indicators, the initial scheme of community diabetes risk assessment system could provide references for the construction of final community diabetes comprehensive management and control model.
