Abstract:OBJECTIVE To construct a well-established rehabilitation service model for stroke patients in the community. METHODS Firstly, it defined what kind of rehabilitation content the community could provide for the community stroke patients through institutional research. Through the group meeting and interviews, it clarified the location of the communi⁃ ty rehabilitation work process of stroke patients and how the rehabilitation services at all levels connected. RESULTS At present, the community could provide a total of 14 types of services for stroke patients, such as acupuncture, moxibus⁃ tion, physical therapy and rehabilitation. Rehabilitation work at all levels after stroke mainly included 8 service links such as obtaining information on stroke patients, telephone confirmation, and appointment time and place for first rehabil⁃ itation assessment. CONCLUSION Based on TCM technology and physical therapy community stroke rehabilitation servic⁃ es can not only achieve the fundamental purpose of stroke rehabilitation, but also have economic and effective character⁃ istics. The four-level rehabilitation service process of community stroke patients based on various medical personnel and rehabilitation resources in the community had certain advantages, and provided a reference tool for the implementation of stroke rehabilitation work in community health service centers under the hierarchical diagnosis system.
