小儿轮状病毒性肠炎患儿血清1,25-(OH)2VitD3检测 及其免疫功能状况分析

中文摘要: 目的 研究小儿轮状病毒性肠炎患儿血清1,25-(OH)2VitD3检测及其免疫功能状况,为临床对轮状病毒性肠炎的诊疗 提供依据。方法 选择2017年6月—2019年6月在上海市宝山区仁和医院接诊的230例轮状病毒性肠炎患儿作为研究对象。选 取同期我院收治的 112 例普通细菌性肠炎患儿作为对照组。测定比较两组患儿的降钙素原(PCT)、C-反应蛋白 CRP(CRP)、 AGEs(晚期糖基化终末产物)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、25羟维生素D3(1,25-(OH)2VitD3)、免疫球蛋白A (IgA)、免疫球蛋白G(IgG)和免疫球蛋白M(IgM)水平,并对轮状病毒性肠炎与上述指标的关系进行分析。结果 研究组患儿 PCT、CRP、AGEs和IL-2水平明显高于对照组患儿,1,25-(OH)2VitD3、IgA、IgG和IgM水平明显低于对照组,差异具有统计 学意义(P<0.05)。研究组患儿的CRP、PCT阳性率明显高于对照组PCT、CRP阳性率,差异具有统计学意义(P<0.05)。spear⁃ man相关分析结果显示患儿血清PCT、CRP、IL-2、AGEs水平与轮状病毒感染情况互为正相关(r=0.520,0.236,0.407,0.371, P均<0.05),1,25-(OH)2 VitD3、IgA、IgG和IgM水平与轮状病毒感染情况互为负相关(r=-0.634,-0.517,-0.564,-0.602,P均< 0.05)。结论 25-(OH)2VitD3水平的降低可影响患儿的正常免疫功能,增加轮状病毒感染性肠炎的发生率。有效补充25-(OH)2 VitD3可为轮状病毒感染性肠炎的防治提供新的思路。
Abstract:OBJECTIVE The detection of serum 1,25-(OH)2VitD3 and its immune function in children with rotavirus enteritis provide basis for clinical diagnosis and treatment of rotavirus enteritis. METHODS 230 children with rotavirus enteritis from June 2017 to June 2019 were selected as the study objects. 112 children with bacterial enteritis were selected as control group. The levels of procal⁃ citonin (PCT), C-reactive protein (CRP), IL-2 (IL-2), IL-4 (IL-4), 25 hydroxyvitamin D3 (1,25-(OH)2VitD3), immunoglobulin A (IGA), immunoglobulin G (IgG) and immunoglobulin M (IgM) IgM were measured and compared between the two groups, and the rela⁃ tionship between rotavirus enteritis and the above indexes was analyzed. RESULTS The levels of PCT, CRP , AGEs, IL-2 in the study group were significantly higher than the control group, and the levels of 1,25-(OH)2VitD3, IgA, IgG and IgM in the study group were significantly lower than the control group(P<0.05). The positive rates of CRP and PCT in the study group were significantly higher than the control group(P<0.05). Spearman correlation analysis showed that the levels of PCT, CRP, IL-2, ages were positively correlat⁃ ed with rotavirus infection (r=0.520, 0.236, 0.407, 0371, P<0.05). 1,25-(OH)2 VitD3, IgA, IgG and IgM levels were negatively corre⁃ lated with rotavirus infection (r=-0.634, -0.517, -0.564, -0.602, P<0.05). CONCLUSION The decrease of 25-(OH)2VitD3 level could affect the normal immune function of children and increase the incidence of rotavirus infection enteritis. Effective supplementation of 25-(OH)2VitD3 may provide a new way for the prevention and treatment of rotavirus infective enteritis.
