Abstract:OBJECTIVE To investigate the effects of chronic disease management model based on the concept of “Internet plus” in patients with chronic obstructive pulmonary disease（COPD）. METHODS Referred patients with COPD （100 cases） was selected from medical department of Minhang Hospital， Fudan University， and were divided into the experimental group and the control group using random number table. The experimental group received hospital-community integrated chronic disease management model with “Internet plus”，while the control group was given routine COPD management. Six months after intervention，self-management ability，quality of life，health literacy and other indicators of two groups were evaluated and compared. RESULTS Forty-four pa⁃ tients in the experimental group and forty-five patients in the control group completed the study. Six months after intervention， self-management ability，quality of life and health literacy of two groups were improved with different levels. Self-management abili⁃ ty，quality of life and health literacy in the experimental group were better than those in the control group（P＜0.05）. CONCLUSION Hospital-community integrated chronic disease management model based on the concept of “Internet plus” can effectively improve self-management ability，quality of life and health literacy of the patients with COPD and give a positive impact on them.
