Abstract:Abstract: In 2023, the National Health Commission and other departments jointly issued The Work Plan for The Pilot Construction of Close-Knit Urban Medical Groups, requiring the integration and optimization of medical resources based on functional positioning. In 2023, in accordance with the Work Plan for the Pilot Construction of Close-Knit Urban Medical Groups in Minhang District, Fudan University Affiliated Minhang District Central Hospital took the lead in establishing the Minhang South Science and Technology Innovation Center Medical Group. The medical group includes six community health service institutions within its jurisdiction, such as the Minhang District Jiangchuan Community Health Service Center. In 2024, the Minhang South Science and Technology Innovation Center Medical Group began to pilot the integration of the Fifth Hospital and Jiangchuan Community Health Service Center, setting up an integrated ward in the Minhang District Jiangchuan Community Health Service Center with a medical model of“Rehabilitation + General Practice” . During the operation, some experiences were gained, but also some challenges were encountered. It uses the SWOT analysis method to analyze the construction of the integrated ward of the Fifth Hospital and Jiangchuan in the Minhang South Science and Technology Innovation Center Medical Group and proposes corresponding suggestions and countermeasures.
