2013—2023 年上海市青浦区 手足口病流行特征和病原变迁 *

中文摘要: 摘 要:目的:分析 2013—2023 年上海市青浦区手足口病流行特征和病原谱变化,为制定防控策略提供科学依据。方 法:通过国家疾病预防控制信息系统、历年病原监测报表,收集青浦区手足口病病例和病原监测资料,采用描述性流行病学 方法分析三间分布特征和病原监测结果。结果:2013—2023 年青浦区年均报告发病率为 156.33/10 万,发病率总体呈下降趋 势 ( χ 2 =4 314.73,P<0.01)。2013—2017 年发病呈双峰 (4—7 月和 10—12 月),其中 2016—2017 年夏季后移 (6—7 月), 2018—2019 年和 2023 年呈单峰 (6—10 月),2020—2022 年持续低发。地区发病率依次为青东片区>城区>青西片区。年龄 分布以 5 岁及以下儿童为主,占 87.17%;散居儿童占 58.57%,幼托儿童占 31.63%,学生占 9.16%,差异有统计学意义 ( χ 2 = 1 086.80,P<0.01),2023 年学生病例占比超过幼托儿童。CoxA6 检出阳性率上升并成为优势病原 ( χ 2 =80.48,P<0.01)。 结论:6—10 月是青浦区手足口病发病高峰时间,应对 5 岁及以下儿童、托幼机构和学校尤其是青东片区和城区加强手足口 病防控工作,加强病原监测工作,掌握手足口病流行特征。
Abstract:Abstract: OBJECTIVE To analyze the epidemic characteristics and pathogenic spectrum changes of hand, foot and mouth disease in Qingpu District from 2013 to 2023,and provide scientific basis for formulating prevention and control strategies. METHODS Hand, foot and mouth disease cases and pathogen surveillance data in Qingpu District were collected through the National Disease Prevention and Control Information System and pathogen surveillance reports over the years, and descriptive epidemiological methods were used to analyze the three-dimensional distribution characteristics and pathogen surveillance results. RESULTS The average annual incidence rate of Qingpu District from 2013 to 2023 was 156.33/10 5 ,and the incidence rate showed an overall downward trend (χ 2 =4314.73, P<0.01). There were double peaks from 2013 to 2017 (April to July, October to December),with the summer peak shifting backward from 2016 to 2017 (June to July),single peaks (June to October) in 2018, 2019,and 2023,and continued low incidence from 2020 to 2022. The order of regional incidence rates is Qingdong area>urban area>Qingxi area. The age distribution is mainly among children aged 5 and below,accounting for 87.17%. The main population is classified as scattered children accounting for 58.57%,kindergarten children accounting for 31.63%,and students accounting for 9.16% , with statistically significant differences (χ 2 =1086.80, P<0.01). In 2023, the proportion of students exceeded that of kindergarten children. The positive rate of CoxA 6 detection increased as the dominant pathogen (χ 2 =80.48, P<0.01). CONCLUSION June to October is the peak period for the incidence of hand, foot and mouth disease in Qingpu District. It is necessary to strengthen the prevention and control of hand,foot and mouth disease in children under 5 years old,kindergartens, and schools, especially in the Qingdong area and urban areas. Strengthening pathogen monitoring can help grasp the epidemic characteristics of hand,foot and mouth disease.
