中文摘要: 摘 要：目的：调查1例在接种麻腮风联合减毒活疫苗(MMR) 后出现麻疹症状的儿童病例，明确其病因并评估疫苗安 全性。方法：采用个案调查结合实验室检测方法，对该预防接种异常病例进行全面调查分析。实验室检测包括麻疹免疫球蛋 白 (IgM) 抗体检测、核酸检测以及基因测序，以确定病例的病原性质。结果：北京市丰台区2024年报告1例接种MMR后 出现麻疹症状的病例。该患儿在接种MMR后第6天出现发热，接种后第10天颜面部及躯干部出现散在皮疹。出疹后第一天 分别采集静脉血和咽拭子，检测结果显示，麻疹IgM 抗体和风疹IgM抗体均为阴性，麻疹核酸阳性，基因测序分析结果确认 为疫苗株。结论：该病例为疫苗株感染所致。通过结合流行病学调查和实验室检测结果，能够快速地鉴别疫苗相关麻疹病例， 进一步提高麻疹风疹网络实验室运行的敏感性，为疫苗安全性监测提供重要依据。
Abstract:Abstract: OBJECTIVE To investigate a case of measles symptoms in a child following administration of the measles-mumps- rubella(MMR)vaccine,determine the cause,and evaluate vaccine safety.METHODS A comprehensive investigation and analysis of the adverse event following immunization(AEFI)case was conducted using a case investigation combined with laboratory testing methods.Laboratory tests included measles IgM antibody detection,nucleic acid testing,and gene sequencing to determine the pathogen nature.RESULTS One case of measles-like symptomsfollowing MMR vaccination was reported in Fengtai District of Beijing in 2024.The child developed fever on day 6 post-vaccination and scattered rash on the face and trunk on day 10.Venous blood and throat swab samples were collected on the first day after rash onset.Test results showed negative measles IgM and rubella IgM antibodies, positive measles nucleic acid,and gene sequencing analysis confirmed the vaccine strain.CONCLUSION The case resulted from vaccine strain infection.Integrating epidemiological investigation with laboratory testing enables rapid identification of vaccine- associated measles cases,enhancing the sensitivity of the measles-rubella network laboratory system and providing crucial evidence for vaccine safety monitoring
